Our Services

Patient Information
We will need the parents/guardians of new patients to fill out a “New Patient Information Form” that covers the child’s medical history, current condition, insurance information, allergies and any other medical facts we need to know. Once we have this information, we will then begin the first appointment. Depending on the nature of the appointment and the age of the patient, we will determine if treatment is necessary and recommend steps parents/guardians can take to keep their child healthy. By continuing to keep up regular visits, we can monitor the child’s growth and identify unhealthy changes that occur. We will help to ease the child into an appointment, so the child is comfortable visiting the doctor.
